New Patient Registration
Thank you for your interest in registering to become a patient at Clive Medical Practice. Unfortunately, we are unable to accept any patients that are currently living in Wem who are already registered with a Medical Practice within their boundary area.
Due to capacity, we can only register patients that have moved into the Wem area who are in need of registering with a GP Practice that is within the boundary area for their home address.
We know that this may be disappointing for you, but if you are having trouble accessing appointments at your registered GP Practice you will need to contact them directly so they can advise.

If you do not wish to register online please speak to reception and they will provide you with a form for completion. The forms can also be downloaded from the above "Registering Online" tab. Once completed we can then request your notes from your previous practice.
Patients register with the practice rather than with a particular doctor. If you do have a preference of which GP you see this can be recorded when you register. You can also ask to see a particular doctor when making an appointment. This will be accommodated where possible, but it may mean waiting longer for an appointment.
Medical treatment is available from the date of registration. Please contact reception for further information.
Online 'Pre-Registration'
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Accountable GP
Allocation of Accountable GP
By March 2016, it is a requirement of all GP surgeries to allocate an accountable GP to each registered patient. This process is already underway and over the coming months we will be informing all patients, of who their accountable GP is.
The named accountable GP is the doctor who within the surgery has been chosen to oversee and look after their patients by liaising with all the rest of his or her colleagues to make sure that the best care possible is given to them.
Your named accountable GP may not be your doctor that you are regularly seeing
Regardless of who your accountable GP is, patients are still welcome to see any GP within our practice.
